MISSOURI'S FALLEN WORKERS

These are a few of the men and women that went to work in Missouri and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork. We would like to sincerely thank the families that shared their loved one's picture and their tragic loss with USMWF.

 

peopleBrian D Allen, 49

Brian was killed on the job in October 2013 while working for Larry Strate Plumbing & Heating (Strate Plumbing, Holden Mo) in Lees Summit, Missouri. He was installing a residential sewer line to the city sewer and died in a 17ft trench collapse.

Federal OSHA investigated the incident and proposed a $35,000.00 penalty for five serious violations. In an informal settlement the penalty was reduced $5,600.00 penalty for two serious violation. This inspection is closed.

peopleHoyt Ferguson, 46

Hoyt was killed on the job in June 2012 while working for MG Dyess in DeKalb, Mississippi (He resided in Willard, MO). He died when the pipes in the back of the truck smashed into the vehicle's cab.

Federal OSHA investigated the incident and proposed no penalty or violation. MG Dyess was issued a Hazard alert with recommendations to protect employees from struck by hazards. This inspection is closed.

peopleVincent Don Lavite, 38

Vincent was killed on the job in April while working at Holcim, Ste. Genevieve Cement Plant in Missouri. He died from a fall.

MSHA investigated and proposed a $5,000.00 penalty for not contacting them within 15 minutes of the incident.

peopleSteven Michael Lillicrap, 21

Steven was killed in February 2009 while working for Ben Hur Construction in St. Louis, Missouri. He died while disassembling a 100 ton crane.

MO OSHA investigated the incident and proposed a $84,000.00 penalty for two serious violations and one willful violation. There were no reductions.

peopleMichael Steele, 15

Michael was killed on the job in July 2013 while working for John Cragen (Family Farm) in Frankford, Missouri. He died after he fell off the tractor he was driving and was ran over.

This is not in OSHA's jurisdiction.

---- He was a very hard worker for a 15 y/o. Besides hauling hay, he also worked all winter cutting and splitting firewood to sell and he had 3 gardens of produce he had planted to sell to make money for the truck he wanted. Never got the chance to sell it though. He had a great sense of humor and I've never heard anyone say a bad thing about him or knew of anyone who didn't like him. He had also been dating the same girl since they were 13 but sadly never made it to their 2 year anniversary on August 2. To see more about Michael. - To see article.

peopleJacob Von Kopfman, 21

Jacob was killed on the job in August 2011 while working for Hayden Tower Service Inc in Brookfield Missouri. He fell 84 feet while dismantling a tower.

Federal OSHA investigated the incident and proposed a $ 9,800.00 penalty for two serious violations. In an informal settlement the penalty and violations were deleted. The Case is closed.

peopleGus A. Santamaria, 29

Gus was killed in August 2014 while working for Blue Springs automotive (Highline Autosport) shop Blue Springs, Missouri. He died when a four-post vehicle lift tipped over and fell on him.

Federal OSHA investigated the incident and proposed a $19,600.00 penalties for seven serious violations. In an informal settlement the penalties and citations were reduced to $5,150.00 for two other than serious violations.
