Education and Activism Opportunity!!

H and S community in Denver on Nov. 4-7

USMWF was able to bring 13 family members to attend the National COSH meeting the Health and Safety Summit and meet with an amazing team at the Denver OSHA office, Region 8 . The Denver visit was a very unique because USMWF members have never meet with a Labor Liaison or a Solicitor of Labor.  USMWF has enjoyed many events and meetings with OSHA offices but felt this particular visit was a good start for new relationships and very informative.

Robert Augeri, 31

Maurice Jay Alexander, 40 ~ 12/22/1969 - 8/27/2010 ~ Sent in by wife, Mary Ann Neely Alexander ~ Maurice lay trapped beneath an 800-pound bale of cardboard. He had been using a forklift to move the bales, and when he jumped down to adjust the machine, one came crashing down on top of him. No one knows how long he, he was struggled to stay alive at Sonoco Recycling.

Robert Augeri, 31

Robert Augeri, 31 ~ 6/3/1977 - 6/3/2008 ~ Sent in by wife, Kim Augeri ~ Killed at Brox Industries when struck by truck in Dracut, MA, Labor Union Local 175.  To view tribute click here.


Robert Augeri, 31 was killed on the job on 06/03/08 while working at Brox Industries in Dracut, MA. He died after being struck by a truck. Federal OSHA investigated the incident and assessed a $14,700 penalty. The penalty was reduced to $10,000.

Craig Bagley, 33

2/5/1979 - 7/19/2007 ~ Sent in by Michele and Tom Kahle ~ Killed at Alaska Mechanical Inc. when JLG120HX Man Lift tipped over.

Paula Bellamy, 38

1973 - 3/28/2011 ~ Sent in by daughter Prencess Bellamy ~ Killed when struck by fork truck what working for for Ceres Terminals Incorporated at APM Terminals Virginia in Portsmouth VA. 3/28/2011. OSHA investigated the incident and assessed a $13,200 penalty, this case is not closed.

Gregory Robert Bish, 26

Gregory Robert Bish, 26 ~ 9/6/1984 - 12/9/2010 ~ Sent in by Mother and Father, Terrance and Kimberly Bish ~ Killed at West Penn Energy Services when thawing a valve on the back of the empty water tanker in the yard of the facility when the explosion occurred causing a manhole cover to come off a residual waste tanker truck, striking Gregory, in Elderton,PA.

Stephanie Michelle Brown, 17 ~ 7/28/1989 - 3/16/2007 ~ Sent in by grandfather, Richard A. Sands ~ Killed at Atlantis Strip-Club.

Shawn D. Boone, 33

Shawn D. Boone, 33 ~ 6/27/1970 - 10/30/2003 ~ Sent in by sister, Tammy Miser ~ Killed at Hayes Lemmerz Int. in an aluminum dust explosion. To view full tribute click here.


Shawn D. Boone, 33 was killed on the job on 10/30/03 while working at Hayes Lemmerz Int. in Huntington, IN. He died from severe burns due to aluminum dust explosions. Indiana OSHA investigated the incident and assessed a $42,000 penalty. The penalty was not reduced.

Marlo Paul Cabot, 41

Cody James Campbell, 28 ~ 1/17/1983 - 3/15/2011 ~ Sent in by parents, Jay and Joanna Campbell ~  Cody lives in Minnesota and was injured on a job site while working in Iowa for Railworks Track Systems and died from his injuries a week later in Rochester, Minnesota. While a work team was pounding in rail spikes, a piece of metal split off the rail and ricocheted backwards 20 feet and struck Cody in the neck, while he was working on something else. The piece of metal severed a facial artery and lodged in the floor of his mouth. We feel it was a freak accident, but it is our opinion it was the lack of quick response of medical care that caused his death. He got to the emergency room in less than 10 minutes, but they made him sit down and fill out paperwork before seeing him, not realizing the gravity of his injury. So, we feel it is the hospitals worry of whose paying that cost us our son’s life. We did find out that Railworks was issued two Citations from Iowa OSHA for serious violations. One for not furnishing and a place of employment free from recognized hazards that could likely cause death or serious physical harm to employees and the other for hand tools or equipment were not kept in a safe condition. The proposed amount of both citation penalties is $3,750.

Jin Chen, 22

Jin Chen, 22 ~ 1/3/1987 – 1/26/2009 Sent in by Parents, Shan Chen and Lichun Lai ~ a force for beauty and inspiration in this world Jin was dearly loved and will be greatly missed. ~ Killed in fall from building while working at Microsoft, Bellevue, WA. To view tribute click here.


Jin Chen, 22 was killed on the job on 01/26/09 while working at Microsoft Headquarters in Bellevue, WA. He fell from the 28th floor. The Washington State OSHA office in Bellevue decided not to conduct an investigation.

Dann Collins, 47 ~ 10/16/63 - 8/24/2011 ~ Sent in by wife, Debbie Collins ~ Killed while on call for Big Valley Towing when an oversized load struck him, AZ

Emilio R Deleon, 19

Emilio R Deleon, 19 ~ 5/13/1991 - 8/16/2010 ~ Sent in by parents, Albert and Diane DeLeon ~ Killed by electrocution in a construction incident while working at Scarborough Construction in Grand Island, NE


Federal OSHA investigated the incident and assessed a $23,800.00 penalty. The penalty was reduced to $16,660.00.

Chad Cook

Chad Cook, 25 ~ Sent in by mother, Gay Cook ~ Killed while at Metikki Mine in Hyndman, PA while driving a loaded coal haulage truck for Savage Services at the Metikki Mine.


Chad Cook, 25 was killed on the job on 11/08/05 while working for Savage Services at the Mettiki Mine in Maryland. He died in a coal truck crash. MSHA failed to investigate the incident until it was brought to their attention by the victim's mother and the Charleston Gazette. No citations or penalties were issued.

Vincent Don Lavite, 38

Vincent Don Lavite, 38 ~ 12/14/1970 - 4/14/2009 ~ Sent in by brother, Bradley A. Lavite ~ Vince suddenly passed away in a tragic work related fall while working at Holcim, Ste. Genevieve Cement Plant in Missouri ~ Wood River, IL Union Local 338 and working for ThyssenKrupp Safway

Steve Durbin, 57 was killed on the job on 9/15/2003 while working at Kokosing Construction in Heath, OH. He died from a trench collapse. Federal OSHA investigated the incident and assessed a $15,000 penalty. The penalty was reduced to $7,500.
