Education and Activism Opportunity!!
H and S community in Denver on Nov. 4-7
USMWF was able to bring 13 family members to attend the National COSH meeting the Health and Safety Summit and meet with an amazing team at the Denver OSHA office, Region 8
. The Denver visit was a very unique because USMWF members have never meet with a Labor Liaison or a Solicitor of Labor. USMWF
has enjoyed many events and meetings with OSHA offices but felt this particular
visit was a good start for new relationships and very informative.

Robert Fitch, 51 (Uncle Bobby) was killed on the job on 01/29/09 while working at
Archer Daniels Midland in Lincoln, NE. He died from a fatal fall. Federal OSHA
investigated the incident and assessed a $12,500 penalty. The penalty was reduced to $0.00.

Matthew John Ford, 26 was killed on the job on 10/27/02 while working at Southland Electric Supply in Burlington, NC . He died from a fatal fall. North Carolina OSHA investigated the incident and assessed a $5,000 penalty. The penalty was reduced to $4,000.

Jeremy Foster, 19 was killed on the job
on 11/04/04 while working at Deltic Timber in Ola, AR. He died after being pulled into an
auger. Federal OSHA investigated the incident and assessed a $4,500 penalty. The penalty
was reduced to $2,250.

Raymundo C. Gonzalez, 54 was killed on the job
on 11/12/04 while working at BP in Texas City, TX . He and another worker died after an
explosion. Federal OSHA investigated the incident and assessed a $109,500 penalty. The
penalty was reduced to $1,500. (Less than 6 months later, another explosion occurred at
this same worksite, killing 15 workers.)

Mark Allen Gray ~ Sent in by Aunt Carol Confere ~ Truck rollover ~ Catenary Coal Co. Tribute by his wife Bevery McCarty Gray: Mark's life was tragically ended working as a surface miner in WV when his articulated truck rolled into a sediment pond, entrapping him and he drowned. Mark was a very compassionate person who loved life and was loved by many. He loved children and for years, along with his wife, taught Sunday School for several years. Mark was an avid
fisherman and loved to hunt. He was a beautiful person, inside and out and many
lives were changed due to this preventable accident.
Mark Allen Gray, 27, died on 7/28/2009 at the Samples Mine in Eskdale, WV while working for Hawkeye Contracting. He died from downing after the truck he was driving overturned. MSHA investigated and issued one citation; a civil penalty has not yet been assessed by MSHA.

George "Junior" Hamner, 54 died on
1/2/06 at the International Coal Group's Sago Mine in Sago, WV. He and 11 other men died from carbon
monoxide poisoning. MSHA investigated the disaster and did not identify any contributing violations
and assessed no penalties.

Christopher Jaymes Hansen was killed on the job on 2/2/2006 while working at Jennie-O-Turkey Store in Melrose, MN . He was fatally crushed by a piece of equipment. Minnesota OSHA investigated the incident and assessed a $31,000 penalty. The penalty was not reduced.

Stevie was born March 20,1970. He died on Friday January 20, 2012. He was working for Energy Drilling Company out of Natchez, MS.
According to the only report we have seen (why by the way was in the Longview, TX newspaper) Stevie fell 50 feet from the derrick to the floor of the rig. He was working on Rig number 1, for Energy Drilling Company, Natches, MS on Rig #1 located near Atlanta, TX. My husband and I are working on this together, and we were his brother and sister in law.

Monique Harper, 41 ~ 1/20/1969 - 8/31/2010 ~ Sent in by Niece, Danielle Harper ~ Monique Harper,
41 was found buried in a rice storage facility on Tuesday. The Stuttgart Fire
Department received the call about 5:30 a.m. When fire crews arrived on the scene, co-workers were attempting to dig
the trapped employee out. Fire crews eventually located Harper. Paramedics tried to revive Harper, but she was
pronounced dead at a local medical facility at 6:28 a.m. According to a police report Harper's co-workers became
concerned about her whereabouts when they were unable to contact her by radio. That's when employees began searching
for Harper. Killed at Producers Rice Mill in Stuttgart, AR

Scott Heilert, 36 was killed on the job on 9/9/2004 while working at Custom Structural Inc
(iron worker) in Pewaukee, WI. He died after being struck by a vehicle on a construction
site. Federal SHA investigated the incident and assessed a $3,000 penalty. The penalty
was not reduced.

Jeffery Alan Isaac Jr, 31 was killed on the job on 4/18/2007 while working at JE Higgins Lumber Warehouse in Reno, NV . He died from a Crushed by load of lumber. NV OSHA investigated the incident and assessed a $11,200 penalty. The penalty was reduced to $5,600.

he was attacked by two teens and had a heart attack, he worked for Nelson Trucking Company,
Mountain City, TN.
Brian Steven Isdell, 55 was killed on the job on 8/20/09 while working at Nelson Trucking in Mountain City, TN . He died from a heart attack following a violent attack by criminals. Federal OSHA investigated the incident and assessed a $280 penalty. The penalty was not reduced.

traveling at 50 while he was working for the Anderson Indiana Street Department.
Dale Jones, 44 was killed on the job on 12/6/09 while working at Anderson Indiana Public Works (Street Dept) in Anderson, IN. He died after being struck by a car. Federal OSHA investigated the incident but did not issue any citations or penalties

Lee Joslyn ~ Murdered by a coworker and a disgruntled former employee in a planned robbery and
intentional double homicide. Found in the Arby's cooler/freezer an hour or so after closing time;
shot multiple times.
Nicole Chartre' Joslyn, 24 was killed on the job on 10/24/04 while working at Arby's in West Milwaukee, WI. She was murdered by a disgruntled employee. OSHA was unable to provide information on this case
because they do not investigate homeicideds.

William Knorr, Jr 32 was killed on the job on 06/26/04 while working at Grant Tower (cell phone tower) in St. Ignace Township, MI. He died from a fatal fall. Michigan-OSHA investigated the incident and assessed a $98,000 penalty. The company contested the penalty and the case is still pending.
