Adrian was killed on the job in April 2015 while working at Nebraska Railcar Cleaning Services, Llc in Nebraska. He died from an explosion.

OSHA investigated and proposed a $963,000 penalty for 20 serious, 10 willful, 2 repeat, and one other than serious violation. The case is still currently open.

peopleVincent Don Lavite, 38

Vincent was killed on the job in April while working at Holcim, Ste. Genevieve Cement Plant in Missouri. He died from a fall.

MSHA investigated and proposed a $5,000.00 penalty for not contacting them within 15 minutes of the incident.

peopleChristopher “Chip” Leasure, 45

Christopher was killed in January 2016 while working for BJ Baldwin Electric, at  East Penn Manufacturing, in Lyons Station, PA. Christopher was killed when he came in contact with a high-voltage wire and was electrocuted.

Federal OSHA is investigating the incident and the case is still open.

peopleColey Gene Lee, 24

Coley was killed in May 2012 while working for Long Brothers Oil Company in El Dorado, Arkansas. Coley was killed when he was disassembling an old oil tank. The fumes ignited causing and explosion. Mercil "Shane" Chelette, 38 and Lee Randal Ross, 24 were also killed in the explosion. 

Federal OSHA investigated the incident and proposed a $4,800.00 penalty for two serious and one other than serious violation. There were no reductions.

peopleJimmy Lee, 33

Jimmy was killed in May 2006 while working at Kentucky Darby Coal Mine in Harlan County, Kentucky. He was killed in a methane explosion.

MSHA investigated and proposed a $336,000 penalty. The company is delinquent in paying the fine; the mine is abandoned.

Steven was killed in February 2009 while working for Ben Hur Construction in St. Louis, Missouri. He died while disassembling a 100 ton crane.

MO OSHA investigated the incident and proposed a $84,000.00 penalty for two serious violations and one willful violation. There were no reductions.

peopleJoshua Matthew Loux, 41

Joshua was killed in May 2014 while working for Restoration & Waterproofing Contractors in Wichita, Kansas. He died when he was ordered on a "for freight only elevator" and his head was caught between the elevator and cement abutment from the floor above.

Federal OSHA investigated the incident and proposed a $7,000.00 penalty for one serious violation. In an informal settlement the penalty was reduced to $7,000.00 for one other than serious violation.

Alan was killed in October 2004 while working for Arby's in West Milwaukee, Wisconsin. He was shot while working.

This is not in OSHA's jurisdiction.

This photo was sent in by a family friend, Lee Joslyn. ----Alan was killed on the job on 10/24/04 while working at Arby's in West Milwaukee, WI. Murdered by a co-worker and a disgruntled former employee in a planned robbery and intentional double homicide. He was found in the Arby's cooler/freezer an hour or so after closing time; shot multiple times.

Walter was killed in October 2006 while working at Tilcon, Inc. in Wharton, New Jersey. He died after being crushed by a machine.

PEOSH investigated the incident and proposed a $12,500.00 penalty for two serious violations and one willful violation. In an informal settlement the penalty was reduced to $7,500.00. This photo was sent in by his wife, Karen.

peopleTravis Lee Lunn, 24

Travis was killed in September 2008 while working for John Prouty Construction Co. in (O'Neil) Verdel, Nebraska. He died when a trench collapsed,

Federal OSHA investigated the incident and proposed a $201,600.00 penalty for three willful violations and two serious violations. In an informal settlement the penalty was reduced to $105,000.00 with four serious violations and one willful violation.

Richard was killed in February 2007 while working for New Orleans Hotel Casino/Boyd Gaming in Las Vegas, Nevada. He died from affixation in a confined space.

NV OSHA investigated the incident and proposed a $185,000.00 penalty for eight serious violations and one other violation. This penalty was not reduced.
