peopleRussell Mann, 45

Russell was killed in September 2012 while working for Wynnewood Refining Company (CVR Energy Inc.-owned plant) in Wynnewood, Oklahoma. He was killed when a boiler exploded. Billy Guy Smith, 34 was killed in the same incident.

Federal OSHA investigated the incident and proposed a $234,500.00 penalty for eleven serious violations, five repeat violations and one other than serious violation. The case is still open.

peopleJuan Pablo Marillo, 30

Juan was killed in January 2005 while working for Acetylene Service Company in Perth Amboy, New Jersey. He was killed in an acetylene explosion. Two other men were also killed.

Federal OSHA investigated the incident and proposed a $25,200.00 penalty for ten serious violations. In an informal settlement the penalty was reduced to $21,420.00

peopleGreg Martin, 44

Greg was killed in September 2008 while working for B.L. Myers Bros. in Glenmoore, Pennsylvania. He died when a 34-ton vehicle tipped onto its right side and he was hit by the 25-foot mast.

A investigation into the disaster and did not identify any contributing violations and assessed no penalties.

Anthony was killed on the job in July 2014 while working for Max I. Walker in Omaha, Nebraska. He was killed in a head-on motor vehicle crash near Indianola.

Orestes was killed on the job in April 2009 while working for J.T. Vaughn Enterprises in Houston, Texas. He was crushed when a two-ton door fell on him while installing it at M.D. Anderson Cancer Center.

Federal OSHA investigated the incident and proposed a $10,000. 00 penalties for two serious violations. In an informal settlement the penalty was reduced to $3,500.00 for one serious violation. The company paid the fines and the case is closed.

Benjamin was killed on the job in May 2014 while working at Reading Alloys for Z Weldco in Oley, Pennsylvania. He While he was welding an industrial hopper filled with lime that became unstable and fell on him.

Federal OSHA investigated the incident and proposed a $14,600.00 penalties for five serious and two other than serious violations. In an informal settlement the penalty was reduced to $8,760.00 for five serious and two other than serious The company paid the fines and the case is closed.

Llewellyn was killed in May 2014 while working for Pathfinder Irrigation District in Scottsbluff, Nebraska. He died after falling approximately 11 feet down into a controlled burn in the ditch.

Stacy was killed July 2006 in Tulare, NY when assaulted by her employer.

This photo was sent in by her sister Christy Walls. ----FAUSTON BASSO SENTENCED TO 27 YEARS AND 8 MONTHS IN STATE PRISON -On July 20 2009, Tulare County Superior Court Judge Darryl Ferguson imposed a prison sentence of 27 years and 8 months in state prison on 39-year-old Fausto (Mike) Basso of Garden City, New York. On June 4, 2009 the defendant plead no contest to voluntary manslaughter, assault with a deadly weapon, and residential burglary. He admitted that he personally used a firearm during the crimes and that he inflicted great bodily injury. The defendant killed victim Stacey McKinney age 28 on July 13, 2006 in her home in Tulare. The defendant also inflicted serious injury on victim Andrew McKinney age 29, during the attack. All three crimes are violent felonies or strike offenses under the 'Three Striker' law. Furthermore, his credits are limited to 15% on the entire case. He was ordered to pay over $11,000 in restitution and over $1000 in fines. This case was prosecuted by Deputy District Attorney Afreen Kaelble, and investigated by Detective Todd Davis of the Tulare Police Department and Investigator Brain of the Tulare County District Attorney's Office Bureau of Investigations.

Cody was killed in January 2011 while working for Nine Mile Canyon in Roosevelt, Utah. He died when he was crushed by machinery.

This photo was sent in by his parents Dave and Chris McMullin. ---- January 30, Nine Mile Canyon UT - 32 year old Cody Earl McMillan of Roosevelt, a water truck driver, had been removing water from a pit at a location in the Wells Draw area of Nine Mile Canyon when he climbed onto the pump jack at the well site for some reason, said Duchense County Sheriff's Sgt. John Crowley. McMillan fell into the pump jack and suffered fatal crush injuries, Crowley said. The man's body was discovered by other oil-field workers when they arrived at the site Sunday morning. Chris was killed on the job in October, 2014 while working for Northeast Nebraska Automation, Inc in Thurston, Nebraska. Chris died after he was struck and killed by a load that fell from a crane.

Jerry was killed on the job in August, 2011 while working for Spirt AeroSystems in Wichita, Kansas. Jerry died after he fell 13 feet and suffered traumatic head injurie.

Federal OSHA investigated the incident and proposed a $7,000 penalty for one serious violation.

Jordan was killed in July 2011 while working at (Loren and Jeff Groeneweg) DBA JL Cattle at Inwood, Iowa. He died when the loader lunged forward, crushing Jordan's head against the header of the shed.

IOSH investigated the incident and proposed a $6,000.00 penalty for three serious violations and one other than serious violation. There was no reductions.
