Education and Activism Opportunity!!

H and S community in Denver on Nov. 4-7

USMWF was able to bring 13 family members to attend the National COSH meeting the Health and Safety Summit and meet with an amazing team at the Denver OSHA office, Region 8 . The Denver visit was a very unique because USMWF members have never meet with a Labor Liaison or a Solicitor of Labor.  USMWF has enjoyed many events and meetings with OSHA offices but felt this particular visit was a good start for new relationships and very informative.

Alan H. Reinstein, 66 ~ 1939 - 2006~ Sent in by wife, Linda Reinstein ~ Killed from mesothelioma, Asbestos Disease Awareness Organization.

Kyle Rhodes, 28 ~ 2/14/1983 - 5/19/2011 ~ Sent in by Wife, Jill Rhodes ~ Killed at Fairfield Manufacturing, Lafayette, IN when pinned between a forklift and a semi-truck. 22-year-old Gregory Peterson III, suffered injury in the same work accident.


IOSHA investigated the incident and assessed a $9,000 penalty. Case Closed

Dirk Remington, 46 ~ 4/22/1963 - 12/14/2009~ Sent in by father, Dale Remington ~ Dirk Remington was killed in a tower collapse. He was employed by Demand Communication of Owego, NY. They were in the process of replacing guy wires in a maintenance job. The tower collapsed at approximately 2:30 in the afternoon and the outcome of the OSHA investigation has not been completed. Also Sent by Son Denny Remington, who states he is my father and best friend.

Vicente Enrique Rodriguez, 20

Vicente Enrique Rodriguez, 20 ~ 5/15/1989 - 5/20/2009 ~ Sent in by mother, Marychris Rodriguez ~ Killed in in 36 ft 8in fall, employer failed to provide the required fall protection, and instead provided unsafe oak planking rather than proper catwalks while working at Hollywood Theatre MGM, NV. Vicente


Enrique Rodriguez, 20 was killed on the job on 05/20/09 while working at Hollywood Theatre/MGM in Las Vegas, NV. He died from a fatal fall. Nevada-OSHA investigated the incident and assessed a $38,7000 penalty for six serious and 1 other-than-serious violation. The company contested. The review board deleted three of the serious violations, and assessed a penalty of $19,800.

Doug Seaney, 37

Doug Seaney, 37 ~ 4/28/1972 - 3/12/2010 ~ Sent in by wife, Jacki Seaney ~ suffocated in grain bin while working for Herrmann Grain Farms, Hinckley Il.

Sheri Sangji, 23

Sheri Sangji, 23 ~ 11/2/1985 - 1/16/2009 ~ Sent in by best friend, Zahra Khan ~ Killed from a fire that started when she was transferring t-butyl lithium - a chemical that combusts on contact with air while working in a UCLA lab. Sheri Sangji, 23 was killed on the job on 01/16/09 while working at University of California Los Angeles in Los Angeles, CA. She died in an explosion caused by the reactive compound t-butyl-lithium. CAL-OSHA investigated the incident and assessed a $31,875 penalty.

Austin J. Sawicki, 26

Austin J. Sawicki, 26 ~ 3/24/1982 - 5/29/2008 ~ Sent in by mother, Mary Aloi ~ Killed when a steel beam was dropped on him while working for Whillock Construction. We were waiting for him to come to the hospital as his sister was giving birth to his nephew Wesley Austin Zimmer. He is missed so much. We will miss you forever and for always.


Austin J. Sawicki, 26 was killed on the job on 05/29/08 while working at Whillock Construction in Chula Vista, CA. He died after being struck by a steel beam. CAL-OSHA investigated the incident, and assessed $13,950. The penaties was reduced to $5,150.

Scott Shaw, 38

Scott Shaw, 38 ~ 7/13/1964 - 10/7/2002 ~ Sent in by wife, Holly Shaw ~ Killed at Unstated Construction from fall in river while working on Schuykill River.


Scott Shaw, 38 was killed on the job on 10/07/02 while working at Armco (Weeks Marine contracted w/ Armco) in Schuykill River, PA. He drowned. Federal OSHA investigated the incident and assessed a $4,950 penalty. The penalty was reduced to $0.00.

Joel Ethan Schorr, 38

Joel Ethan Schorr, 38 ~ Feb. 25 1972 - August 12, 2010 ~ Sent in by wife, Angie Schorr ~ Killed at Barrick Gold of North America- Goldstrike Mine.

Bob Shonkwiler

Bob Shonkwiler ~ 5/7/2003 ~ Sent in by wife, Betsy Shonkwiler ~ Killed at Hoosier Energy when crushed in conveyor belt. To view tribute click here.


Bob Shonkwiler was killed on the job on 5/7/2003 while working at Hoosier Energy in Indiana. He died from crushing injuries from a conveyor belt. Indiana OSHA investigated the incident and assessed a $15,375. The penalty was reduced to $6,465.

Yvonne Shurelds, 58 ~ 8/17/2008 ~ Killed at DTR Industries, OH when found unresponsive.


OSHA investigated the incident and assessed $14,000. The penalty was reduced to $3,500.

Donald Wilcher Smith, 23

Donald Wilcher Smith, 23 ~ 5/27/1982 - 3/26/2005 ~ Sent in by father, Coit Smith ~ Killed at Sanderson Farms when electrocuted. To view full tribute click here.


Donald Wilcher Smith, 23 was killed on the job on 03/26/05 while working at Sanderson Farms in Bryan, TX . He was electrocuted. Federal OSHA investigated the incident and assessed a $38,000 penalty. The penalty was reduced to $12,000.

Erin Sperrey, 20 was killed on the job on 01/02/05 while working at Tim Horton's in Caribou, ME . She was murdered by a disgruntled employee. Federal OSHA investigated the incident and assessed a $4,500 penalty. The penalty was reduced to $2,700.
