H and S community in Denver on Nov. 4-7
USMWF was able to bring 13 family members to attend the National COSH meeting the Health and Safety Summit and meet with an amazing team at the Denver OSHA office, Region 8
. The Denver visit was a very unique because USMWF members have never meet with a Labor Liaison or a Solicitor of Labor. USMWF
has enjoyed many events and meetings with OSHA offices but felt this particular
visit was a good start for new relationships and very informative.

Andreas H. Vionis, 35 ~ 4/19/1971 - 4/27/2006 ~ Sent in by Irene Giles ~ Killed at
KMC Oiltools/Helmerich when electrocuted.
Andreas H. Vionis, 35 was killed on the job on 4/27/2006 while working at
KMC Oiltools/Helmerich in Newton County, TX . He was electrocuted. Federal OSHA investigated the
incident and assessed no citation or penalties.

Fred G. "Bear" Ware, Jr., 59 ~ 1947 - 1/2/2006 ~ Sent in by daughter,
Peggy Cohen ~ Killed in the Sago coal mine explosion, Buckhannon, WV.
Fred G.
"Bear" Ware, Jr., 59 died on 1/2/06 at the International Coal Group's Sago Mine
in Sago, WV. He and 11 other men died from carbon monoxide poisoning. MSHA
investigated the disaster and did not identify any contributing violations and
assessed no penalties.

Wyatt Whitebread, 14 ~ 7/20/2011 ~ Sent in by parents, Gary and Carla Whitebread ~
Killed at Haasbach LLC, in Mount Carroll, Illinois while engulfed by grain. Alex Pacas, 19 was also killed and Will Piper, 20,
injured. OSHA investigated the incident and assessed a $555,000 the penalty was reduced to $200,000. There was and
additional penalty from Wage and Hour of $68,235.

Damien Whipple, 24 ~ 6/15/1985 - 7/10/2009 ~ Sent in by sister,
Kima Whipple-Jackson ~ Killed from fall off a train onto the tracks and was pinned under
another train while switching rail cars at Rescar Industries in Gordon, GA ~ He is deeply
missed by his mother, sisters and brothers, family and friends. We know he is resting with God.
OSHA investigated the incident and assessed a $32,000 penalty. The penalty was reduced to $1600
