These are a few of the men and women that went to work in Ohio and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork.

Patrick Archer, 50
Patrick was fatally injured on the job in November 2021 while working at Ford Motor Company in Lima, Ohio while he was working inside a machine.
OSHA has completed their inspection and the citations are being contested. The case is still considered opened. Inspection: 1562095.015 - Ford Motor Company "Patrick loved without hesitation, without resistance, without reservation, and without prejudice. His love was genuine, sincere, and perfect and was such a blessing to whomever was privileged to experience it. He felt his purpose in this life was to provide for his family and made so many sacrifices to make sure that happened. Taking care of his loved ones and friends was most important to him. He was a man of his word, would light up a room with his smile, and loved with his whole being. He was thoughtful, caring, compassionate, so smart, loving, funny, passionate, and a really bad speller. He was one of the good ones. Words cannot describe how lost I feel without him. Lewis A Bissell, 60
Lewis was killed on the job in July 2012 while working for Superior Beverage Group, Ltd in Solon, Ohio. He was found lifeless on the floor with his forklift nearby in-gear and running.
OSHA investigated the incident and proposed a $3,060.00 penalty for one serious violation. In an informal settlement the penalty was reduced to $1,530.00 for one serious violation. This inspection is closed. Inspection: 527278.015 - Superior Beverage Group, Ltd. We would have been married 35 years in October. His death was a shock! Lewis lived in Mentor, OH. |

Jason E. Gudat, 31
Jason was killed on the job in May 2011 while working at Fairfield Coal (Sutropolis) in Petersburg, Ohio.
This is my son Jason E. Gudat born November 19, 1979 (age 31) has twin sister Jaymie Lynn Gudat Andrews died April 25, 2011 E. Fairfield Coal (Subtropolis) Petersburg, Ohio. I am Jason's mother Karen A. Harris, my husband Arden F. Harris Sr. his father Ronald Gudat grandmother Mildred Loudon he has a twin sister Jaymie Lynn Gudat Andrews a sister Stephanie Ann Gudat Burns step brothers Clyde Harris Arden Harris Jr. Step sisters Elaine Clark Teresa Bostwick Crystal Mounts Joy Shawver many nieces and nephews. |

Gary Henry, 57
Gary was killed on the job in July 2013 while working for Shelly & Sands Inc. in Zanesville, Ohio. He was killed when a dump truck which was loaded with concrete, was backing up when it struck him.
Federal OSHA investigated the incident and assessed a $7,000.00 penalty for one serious violation. The penalty was not reduced and the case is closed. |

Dale Jones, 44
Dale was killed in December 2009 while working for Anderson Indiana Street Department in Anderson, Indiana. He died when he was struck by a car picking up debris.
IOSHA investigated and issued proposed no penalties or citations. Inspection: 313197659 - City Of Anderson Dale was picking up debris from a road after dark and was struck by a car traveling at 50 mph while he was working for the Anderson Indiana Street Department. While working at Anderson Indiana Public Works (Street Department) in Anderson, Indiana. He died after being struck by a car. |

William E. (Willy) Kingrey, 44William was killed on the job in December 2010 while working at Asplunh Tree Expert in Trenton, Ohio. He was killed when he was struck by a CSX train.

Yvonne Von Shurelds, 58
Yvonne was killed in August 2008 while working at DTR Industries in Bluffton, Ohio. She was when found unresponsive.
OSHA investigated the incident and proposed a $14,200.00 penalty for seven serious violations. In an informal settlement the penalty was reduced to $7,100.00 for two serious violations and four other than serious violations. Inspection: 311602361 - Dtr Industries Inc. Our mother, Yvonne Shurelds, 58, was killed on March 8, 2008 while working at DTR Industries in Bluffton, Ohio. She was operating a lift truck, a task that was not part of her regular job duties. She was not certified or properly trained to operate it. There were no witnesses to tell us exactly what happened, but the coroner's report listed her cause of death as internal decapitation. |

Douglas Tout, 44Douglas was killed on the job in September 2013 while working for Joel Cornelius Enterprises in Harrison, Ohio. He fell off a roof and passed away the next day.

Jason T West, 52Jason was killed in December 2014 while working for Hi-Way Paving in Logan, Ohio. He died while he was paving the airport at BWI Airport and was caught in an auger.
Federal OSHA investigated the incident and proposed a $4,450.00 penalty. This case is still open. Inspection: 1011039.015 - Hi-Way Paving, Inc. ---- He was a loving father, son, brother, uncle, friend and husband. He loved mushroom hunting, fishing, and just relaxing with a beer in his hand. I am his daughter Leslie (Rosie) Fraley. Taken from me, I'm sitting here in my room, looking at your picture. Wondering why you couldn't be a part of my future. Uncontrollable tears stream down my face, while my heart beat starts to race. Asking God why he took you from my life, it was more painful than stabbing me in the heart with a knife. I still needed you here you were the one to make everything so clear. you are a part of me and I am a part of you when you died a part of me died too. I never knew how hard it was to lose someone you love until the day you went to heaven above. Even though I can't see, I know your up there watching over me.
