FLORIDA'S FALLEN WORKERS
These are a few of the men and women that went to work in Florida and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork. We would like to sincerely thank the families that shared their loved one's picture and their tragic loss with USMWF.

Timothy Dubberly, 58Timothy was killed on the job in July 2016 while working for Kinder Morgan of Fernandina Beach, Florida. He was killed when the cab of the loading crane he was in separated while he was completing electrical work causing him to fall 100ft to the ground.
USMWF is currently researching additional information regarding this incident. |

Ossie L. Hyman, 47
Ossie was killed in October 2011 while working for Port Of Miami River Terminal, LLC. in Miami, Florida. He died when the platform he was standing on failed, he fell 18ft. and a container crushed him.
Federal OSHA investigated the incident and proposed a $14,700 penalty and three serious violations. In an informal settlement the penalty was reduced to $11,025.00 for one three serious violations. The company paid the fines and the case is closed. Inspection: 315357665 - Port Of Miami River Terminal, Llc. Employed at the Port of Miami for 21 years killed when he was unloading a container on a ship called “The Spirit” when the container crushed him. |

Nicholas Nordin, 27
Nicholas Nordin was injured on the job in May, of 2022 while working for Alpha Foundations Specialists, Llc., in Floral City, Florida. At 8:50 p.m. on May 7, 2022, Employee #1 and Employee #2 were performing foundation repair work on a residential wooden frame house as part of a 2-man work crew . Both employees were working under the crawl space of the house to level the sagging foundation using two jacks. While the two jacks were simultaneously raisin g a galvanized steel beam, Employee #1 began receiving an electric shock and could not remove his hands from the jack wrench. When Employee #1 screamed out for help, Employee #2 started crawling towards him to help but was electrocuted and killed. Employee #1 was hospitalized for injuries sustained from the electric shock.
OSHA investigated the incident and proposed a $43,506 penalty for three serious violations. In an informal settlement the penalty was reduced to $30,454 for three serious violations. 1594333.015 - Alpha Foundations Specialists, Llc. Nicholas was 27 years young at the time of the incident. He always worked very hard and was very successful in the career path he was on. Nick and his fiancé were working hard to build a life together and they had already achieved many of the plans they had for their future. Nick was a point of light! Everyone seemed to like and/or love him. Nick was a presence, he always seemed to raise the level of good energy when he entered a room. He was almost always in a good mood and would help others any way he could. |

Dante M. Perales, 30Dante was killed in February 2005 while working as a Police Officer in Orlando, Florida. He died in a car accident.
This is not in OSHA's jurisdiction. His name is Dante Perales and he is loved and missed more than words can ever say. Everyone seen the picture of him in his Police uniform but I wanted to show a different photo of him. I love you Dante and would do anything to have you back. Love Patricia. He died from a gunshot wound. |

Christopher Ricci, 27
Christopher was killed in December 2014 while working for D.L.S. Prestressed Inc. in Sunrise, Florida. He died when he was crushed by a crane traveling over uneven terrain lost it's balance and toppled over.
Federal OSHA investigated the incident and proposed a $9,800.00 penalty for two serious violations. In an informal settlement the penalty was reduced to $5,880.00 for one serious violation and one other than serious violation. This case is closed. Inspection: 980492.015 - D.L.S. Prestressed Inc. |

Nick Rouskey, 60
Nick was killed on the job in December 2011 while working for Broadcast Services Tower and Antenna in Cape Coral, Florida. He was electrocuted performing maintenance on a tower.
Federal OSHA investigated the incident and proposed no fines or penalties. The Case is closed. Inspection: 316282177 - Broadcast Services Tower & Antenna, Inc. |

Mike Simonetti, 60
Mike was killed on the job in July 2014 while working for Design Landscaping Management in Winter Springs, Florida. He died three days after the riding lawnmower he was driving flipped over in a pond and he became unconscious.
Federal OSHA investigated the incident and proposed a $14,700. 00 penalty for four serious and one other than serious violations. The company is contesting the fines and penalties. Inspection: 984845.015 - Design Landscaping Management, Inc. My father lost his life at work on July 6, 2014 when the Riding mower he was on overturned into a pond. We don't know exactly what happened, (if he was pinned, if he passed out), but he suffered irreversible brain damage as a result of the drowning. The accident took place on July 3, 2014 and we removed him from Life Support on July 6, 2014. |

James "Jimmy" White, 27
Jimmy White became ill on the job in August while working for Veterans Administration Medical Center in Lake City, FL. He is believed to have contracted the COVID-19 virus while at work.
Allison Williams- "Jimmy was proud to be an RN (Registered Nurse) and cared deeply for his patients. He was lost in the line of duty working as a nurse and fighting the COVID-19 pandemic. In his spare time, Jimmy was a video-gamer and movie buff and enjoyed rooting for his favorite sports teams: Chicago Bears “Da Bears”, Chicago Cubs, and Blackhawks. Jimmy was quick-witted, hilarious, and spoke his mind. Those who knew him best will remember all the times he made us laugh and his signature grin. He was extremely loyal to his closest friends and family and would do anything for them. Jimmy had the biggest heart, cared deeply for his fellow human, and was an incredibly grateful person. He was a best friend to his big sister, Allison, and deeply admired his father, David. He had a special bond with his step-mother Mary and was best buds with his brother-in-law Chris and his long-time best friend Kyle Katona. He adored his “sassy southern sister” and close family friend Madeline Mabile and extended family. He will be missed by countless friends and family. There will forever be an empty space at the table and in our hearts. Rest easy, Jimmy. You will never be forgotten." |
