IOWA'S FALLEN WORKERS
These are a few of the men and women that went to work in Iowa and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork. We would like to sincerely thank the families that shared their loved one's picture and their tragic loss with USMWF.

Cody James Campbell, 28
Cody was killed on the job in March 2011 while working for Railworks Track System in Council Bluffs, Iowa. He was killed when he was struck by a piece of metal and died a week later.
IOSHA investigated the incident and proposed a $3,750 penalty for two serious violations. In and informal settlement the penalty was reduced to $1,875.00 for two serious violations. This inspection is closed. Inspection: 315460253 - Railworks Track Systems. Cody lives in Minnesota and was injured on a job site while working in Iowa for Railworks Track Systems and died from his injuries a week later in Rochester, Minnesota. While a work team was pounding in rail spikes, a piece of metal split off the rail and ricocheted backwards 20 feet and struck Cody in the neck, while he was working on something else. The piece of metal severed a facial artery and lodged in the floor of his mouth. We feel it was a freak accident, but it is our opinion it was the lack of quick response of medical care that caused his death. He got to the emergency room in less than 10 minutes, but they made him sit down and fill out paperwork before seeing him, not realizing the gravity of his injury. So, we feel it is the hospitals worry of whose paying that cost us our son’s life. We did find out that Railworks was issued two Citations from Iowa OSHA for serious violations. One for not furnishing and a place of employment free from recognized hazards that could likely cause death or serious physical harm to employees and the other for hand tools or equipment were not kept in a safe condition. |

Ron Heilig, 52Ron was fatally injured in June 2022 while working for Fusion in Atlantic, Iowa. He lost his life due to heart failure during his shift.
This was not in Federal OSHA's jurisdiction. Ron will forever be missed. |

Andrew Joshua Dean May, 35
Andrew was killed in June 2019 while working for Lone Star Truck And Tire Inc. in Brooklyn, Iowa. He was fatally injured after patching a steel radial tubeless semi-trailer tire and mounting it on a steel wheel. At which time a bead failure on the inside dual caused a release of air that forced the outside wheel to strike him in the head and chest.He was then knocked backwards approximately 13ft, striking the rear area of another semi-trailer.
IOSHA investigated the incident and proposed a $10,604 penalty for four serious violations. The fines and penalties were not reduced this case is closed. Inspection: 1412122.015 - Lone Star Truck And Tire Inc. |

Jordan Ross Monen, 17
Jordan was killed in July 2011 while working at (Loren and Jeff Groeneweg) DBA JL Cattle at Inwood, Iowa. He died when the loader lunged forward, crushing Jordan's head against the header of the shed.
IOSHA investigated the incident and proposed a $6,000.00 penalty for three serious violations and one other than serious violation. The fines and penalties were not reduced this case is closed. Inspection: 315826115 - JL Cattle. To see more about Jordan. |

Gary Lee Olson, 63
Gary was killed in July 2019 while working at Miller and Sons Golf Cars LLC at Britt, Iowa.

Traci Menard Sheibal, 37
Traci was killed in February 2012 in Council Bluff, Iowa. She was killed from asphyxiation when she was overcome by the vapors from a paint stripper containing methylene chloride while preparing a bathtub to be refinished.

Jason "Jay" John Steger, 38
Jay was killed in July 2019 while working for Kendrick Forest Products in Edgewood, Iowa. He was fatally injured when he was stuck in the head and neck area by a felled tree.
IOSHA investigated the incident and proposed a $39,780 penalty for five serious violations. In and informal settlement the penalty was reduced to $20,000 for three serious. This inspection is closed. Inspection: 1416007.015 - Kendrick Forest Products Jay is dearly missed by many. He was a wonderful father, son, brother and uncle. |

Benjamin Raymond Van Wyk, 19Benjamin was killed in August 2019 while working at Wehrle Farms in Lynnville, IA. He died due to a grain auger incident.

William "Billy" George Wilson, Jr., 43Billy was killed in March 2019 while working for Summit Products, Inc. in Des Moines, Iowa. He was believed to have been behind the interlocked guarding when one of the transfers moved, pinning him between the transfer and stationary material rack fatally injuring him.
IOSHA investigated the incident and proposed $8,781 for two serious and one other than violation. This case is closed. Inspection: 1382178.015 - Summit Products, Inc. Billy is really missed by his son Aspen, sister, Patti , and brother Brandon. Along with his father and I. He is also missed by his grandmother’s Jerrie and Sandy . Nephews, Austin, Dacoda. Niece’s Mariah and Brooklynne.also one great nice Mya along with his aunts, and uncles. Plus a big amount of cousins. Billy was an outgoing man. He loved his sports. He loved challenges the best. But most of all He loved playing pool with his teammates. Those pool sticks to him was like gold. He was an out spoken person but loving to others and to those who needed his help. This shouldn’t of never happened to him to him but it did and now we as a family wait for answers.. |
