PENNSYLVANIA'S FALLEN WORKERS
These are a few of the men and women that went to work in Pennsylvania and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork. We would like to sincerely thank the families that shared their loved one's picture and their tragic loss with USMWF.

Gregory Robert Bish, 26Gregory was killed on the job in December 2010 while working for West Penn Energy Services in Elderton, Pennsylvania. He died in an explosion on the job.
Federal OSHA investigated the incident and proposed a $6,000.00 penalty for two serious violations. In an informal settlement the penalty was reduced to $5,000.00 for one serious violation. This inspection is closed. Inspection: 309735447-Ceres Marine Terminal Inc. Oh my son, it was -12 degrees that day. If only the company had told you that there were hydrocarbons in the tank even though you cleaned it every day, I know that you knew not to put heat to that stuff. But, there were no mention of hydrocarbons in your training materials & it was never verbally mentioned. I miss you and think of you and your smile & big bubba hugs everyday, son. How I wished we wouldn't have lost you that way because we didn't even get to see you to say goodbye. You are always on my mind, in my heart and forever young. I love you Gregory. |

Nikolai G. Briggs, 26
Nikolai was killed on the job in March 2014 while working for TnT Oil in Warren, Pennsylvania. He was welding on the top of the tank when the torch he was using caused vapor in the tank to ignite and explode.
Federal OSHA investigated the incident and proposed a $10,200.00 penalty for two serious and one other than serious violations. There were no reductions during the informal settlement. This inspection is closed. Inspection: 963019.015 - Tnt Oil Co. ---- My son, Nikolai G. Briggs, was killed on March 12, 2014 in a workplace accident. He was employed by TnT Oil in Warren, PA. He was born on April 26, 1987. He was 26 years old at the time of his death. We miss him terribly. |

Robert Edward Buskey, Jr., 55
Bob was killed on the job on August 28, 2012 while working for Export Fuel Company in Export, Pennsylvania. He was fatally injured when he was crushed by a pavement roller.
OSHA investigated the incident and proposed a $41,300 penalty initially for 18 violations, dropping it down to 12 serious violations. In and informal conference they later reduced it to a $27,540 penalty for 11 serious and one other than serious violations. Inspection: 599038.015 - Export Fuel Company Inc. Bob was a spiritual person whose family was his life. He was known as “Buck” to many, but his greatest nickname was “Dad.” He was a hero to his wife of over 35 years and their 5 children. Bob was very proud of his 5 grandchildren and his legacy of love continues to grow, now with 11 grandchildren total. He is missed immensely but lives on in the hearts and memories of those who love him. |

Jerod Chapman, 29
Jerod was killed on the job in June 2015 while working for Maccabee Industrial Incorporated in University Park, PA. He was fatally injured after a co-worker unpinned the jib at the base of a hydraulic crane that was not secure at which time he was struck by the jib in the head.
OSHA investigated the incident and no penalties/citations were assessed. Inspection: 1070640.015 - Maccabee Industrial Incorporated Jerod was an avid outdoors man and loved hunting and fishing with friends and family, he was also engaged to be married. |

Chad Cook, 25
Chad was killed in November 2005 while working for Savage Services in Hyndman, Pennsylvania (at the Metikki Mine Maryland). He died while driving a loaded coal haulage truck and it flipped over.
MSHA failed to investigate the incident until it was brought to their attention by the victim's mother and the Charleston Gazette. No citations or penalties were issued. |

Nick Farella, 42
Nick was killed on the job in October 2013 while working for Eastchester Department of Public Works (Town of Eastchester City) in Eastchester, NY. He was on the back of a sanitation truck when it backed up pinning him between a telephone pole and the truck. He died during emergency surgery.
This was not in the OSHA's jurisdiction. |

Anthony M. "Tony" Gabriele, 49
Tony was killed on the job in January 2015 while working for Silvi Concrete (Riverside) in Bristol, Pennsylvania. He was killed when a silo collapse and he was buried under mounds of cement. On January 11th Tony was found after being buried for three days.

Gregory Hock, 65Gregory Hock, 65 was fatally injured after working over 30 years for Carpenter Technology Corporation in Reading Technology, PA. Mr. Hock was diagnosed with stage 4 metastatic cancer, bladder, liver and bone. Within 2 months Gregory had lost over 100lbs and had to use a walker for assistance. He sadly succumbed from his illness approximately 2 months after being diagnosed.
This is out of OSHA's jurisdiction. However it is believed that Mr. Hock as a Grinder for over 30 years without being provided with personal protective equipment was exposed and in contact with toxic chemicals such as biocide (a chemical directly in his machine), asbestos and hex chromium. I hope that OSHA takes these toxins seriously so no other worker or family suffers. My father was not the first, he was one of several in his department to pass from this. Masks and other safety equipment must be worn at all times. I will forever fight for my father. He sacrificed seeing me grow up to make sure we had by working 7 days for over 30 years. Greg Martin, 44
Greg was killed in September 2008 while working for B.L. Myers Bros. in Glenmoore, Pennsylvania. He died when a 34-ton vehicle tipped onto its right side and he was hit by the 25-foot mast.
A investigation into the disaster and did not identify any contributing violations and assessed no penalties. This case is closed. Inspection: 312485246 - B.L. Myers Bros. Of Pa, Inc. |

Gary Puleio, 53
Gary was killed in August 2001 while working for Miadville Redi-Mix Concrete in Meadville, Pennsylvania. He died from a fall.
Federal OSHA investigated the incident and proposed a $6,000.00 penalty for two repeat violations. These were not reduced. Scott Shaw, 38
Scott was killed in October 2002 while working for Weeks Marine contracted with Armco in Philadelphia, Pennsylvania. He was killed when he fell in the Schuykill River.
Federal OSHA investigated the incident and proposed a $4,950.00 penalty. In an informal settlement the penalty was reduced to $0.00. |
