Marlo Paul Cabot, 41Marlo was fatally injured on the job in September 2004 while working for Harmon Inc. at the Washington International Airport in Baltimore, Maryland. He was fatally injured when a 1-ton pallet of glass panels fell on him.
Federal OSHA investigated the incident and proposed a $4,625.00 penalty for one serious violation. In an informal settlement the penalty was reduced to a $2,000.00 for one other than serious violation. This inspection is closed. Inspection: 308017730 - Harmon, Inc. Marlo was lost during a construction accident at the Baltimore -Washington International Airport. He was a member of the Iron Workers Union Local #16. Marlo was of a proud Filipino heritage and enjoyed being with friends and family. Marlo and Michele had been married 18 years and have two beautiful children. Michele has lost half of soul, her children's father, her husband and friend. Michael 16 and Katie 15, had a good relationship that extended that of a father and child. Michael has lost his best friend and Katie her famous father whom she strives to emulate.

Cody James Campbell, 28Cody was fatally injured on the job in March 2011 while working for Railworks Track System in Council Bluffs, Iowa. He was fatally injured when he was struck by a piece of metal and passed away a week later.
IOSHA investigated the incident and proposed a $3,750 penalty for two serious violations. In and informal settlement the penalty was reduced to $1,875.00 for two serious violations. This inspection is closed. Inspection: 315460253 - Railworks Track Systems. Cody lives in Minnesota and was injured on a job site while working in Iowa for Railworks Track Systems and died from his injuries a week later in Rochester, Minnesota. While a work team was pounding in rail spikes, a piece of metal split off the rail and ricocheted backwards 20 feet and struck Cody in the neck, while he was working on something else. The piece of metal severed a facial artery and lodged in the floor of his mouth. We feel it was a freak accident, but it is our opinion it was the lack of quick response of medical care that caused his death. He got to the emergency room in less than 10 minutes, but they made him sit down and fill out paperwork before seeing him, not realizing the gravity of his injury. So, we feel it is the hospitals worry of whose paying that cost us our son’s life. We did find out that Railworks was issued two Citations from Iowa OSHA for serious violations. One for not furnishing and a place of employment free from recognized hazards that could likely cause death or serious physical harm to employees and the other for hand tools or equipment were not kept in a safe condition.

Neil C. Cary, 30Neil was fatally injured on the job in June 2010 while working for Benes Heating and Air Conditioning Service Inc. in Raymond, Nebraska. He was fatally injured when he fell from a loft/balcony elevated area.
Federal OSHA investigated the incident and proposed a $11,500.00 penalty for three serious violation and one other than serious violation. Paul E. Cash, 47
Paul was fatally injured on the job in 2018 while working for Lighting Resources Llc in Johnson City, TN. He was fatally injured while trying to extinguish a fire in a bin that contained recyclable aluminum when it exploded.
Federal OSHA investigated the incident and proposed a $44,250 penalty for 11 serious violations. This inspection is closed. Inspection: 1301866.015 - Lighting Resources Llc Paul was taken from us by an explosion at Lighting Resources. His presence is deeply missed by all of us. There's not a day that goes by that he is not missed. A piece of our hearts will always be with him. His siblings are the forgotten mourners that deal with grieving him daily. We never got to say goodbye. Until we meet again, We LOVE you and MISS you Paul. You will never be forgotten. |

Brian Cellar, 62Brian was fatally injured on the job in June 2009 while working at Wynne Transport Service Inc. in Endicott, Nebraska. He was fatally injured when pinned between two trucks. Brian was preparing to pump a load of liquid asphalt and was pinned between two trucks.
Federal OSHA investigated the incident and proposed a $9,100.00 penalty for one serious violation and one other than serious violation. In an informal settlement the penalty was reduced to $5,005.00 This inspection is closed. Inspection: 311467286 - Wynne Transport Service, Inc. He was preparing to pump a load of liquid asphalt from one parked truck to another. The trucks were parked on an incline. The parking brake was not set on the truck. There were no wheel chocks behind the wheels of the truck he was operating. The truck rolled backwards, and he tried to get into the driver's cab to stop it. The truck pinned him between the two trucks. The employee was taken to the hospital where the he died from the injuries he sustained.

Jerod Chapman, 29
Jerod was fatally injured on the job in June 2015 while working for Maccabee Industrial Incorporated in University Park, PA. He was fatally injured after a co-worker unpinned the jib at the base of a hydraulic crane that was not secure at which time he was struck by the jib in the head.
OSHA investigated the incident and no penalties/citations were assessed. Inspection: 1070640.015 - Maccabee Industrial Incorporated Jerod was an avid outdoors man and loved hunting and fishing with friends and family, he was also engaged to be married. |

Jin Chen, 22Jin was fatally injured on the job in January 2009 while working for Microsoft Headquarters in Bellevue, Washington. He was fatally injured when he fell from a roof.
This case is not in the DOSH jurisdiction. The Washington State OSHA office in Bellevue decided not to conduct an investigation. Jin Chen, 22 was killed on the job on 01/26/09 while working at Microsoft Headquarters in Bellevue, WA. He fell from the 28th floor. A force for beauty and inspiration in this world Jin was dearly loved and will be greatly missed.

Michael Lee Clauson , 63Michael was fatally injured on the job in February 2014 while working for Olmsted Trucking in Bear Creek, Wisconsin. He was fatally injured when a truck slid backwards on ice and pinned him between the truck and a barn wall.
Federal OSHA investigated the incident and proposed no penalties or violations. This inspection is closed. Inspection: 959674.015 - Keith R Olmsted & Sons Inc. My name is Lisa Gretzinger and this is my Dad. Michael Lee Clauson born May 24th,1950. He passed away at the age of 63 on February 21st,2014 from Mechanical Asphyxiation when the milk truck he was driving that day slide backwards on the ice and pinned him in between the truck and the milk house. He was employed by Olmsted Trucking of Bear Creek, WI. Ask anyone who knew my Dad and they will tell you how much they liked Dad. Everyone said his smile would always brighten their day. He loved to talk to anyone, and considered everyone a friend. He was one who was always there to help if he was able. He loved his Farmall 300 tractor, and also loved to collect Pez dispensers. OH and we cant forget his love of mowing the lawn on the farm. We always teased him about that. Mom would call me up and say Guess what Dad is doing and I would say mowing lawn. He served in the Vietnam war, and was awarded the Purple Heart and the Bronze Star. There is so much I could say about my Dad. He was not only my Dad, but he was my friend. On February 21st,2014 the world lost a wonderful man and Heaven received an wonderful Angel.

Raven Bianca Rose Marie Cole, 19Raven was fatally injured in October 2009 while working for Werner Construction in Neligh, Nebraska. She was fatally injured when she was pulled into an auger.
Federal OSHA investigated the incident and proposed a $202,000.00 penalty for three willful violations and one serious violation. In an informal settlement the penalty was reduced to $101,000.00 for one willful violation and one serious violation. This inspection is closed. Inspection: 311468649 - Werner Construction, Inc. Raven loved cooking, cars and art, along with driving around in her car and listening to music. She liked to stay up late and sleep in late. Her goals were to go to school for auto body repair and maybe cooking. She hated ham but loved shrimp, steak and chicken.

Christopher G Conklin, 26Christopher was fatally injured in May 2014 while working for Optimum in Long Island, New York.
Federal OSHA is currently investigating the incident. The case is still open as of 8/29/2015. Inspection: 946580 - Optimum. Christopher was ready for his next step in life looking at house to buy and due to get married in 4 months. We miss him more then words could ever express.

Chad Cook, 25Chad was fatally injured in November 2005 while working for Savage Services in Hyndman, Pennsylvania (at the Metikki Mine Maryland). He was fatally injured when he was driving a loaded coal haulage truck and it flipped over.
MSHA failed to investigate the incident until it was brought to their attention by the victim's mother and the Charleston Gazette. No citations or penalties were issued. |

Trina Cunningham, 50Trina was fatally injured in June 2019 while working for the Department of Public Works employee at the Patapsco Wastewater Treatment Plant. It is believed that she stepped onto two 3 feet by 4 feet elevated metal grates at which time the metal grate gave way causing her to fall into a wastewater pit and drowned.
