James L Gabel, 58James was fatally injured on the job in October 2012 while working for Allied Seed Genetics in Geneva, Nebraska. He was fatally injured when a combine ran over him.
Federal OSHA investigated the incident and proposed a $5,600.00 penalty for two serious violations. In an informal settlement the penalty was reduced to $4,200.00 for two serious violations. This case is closed. Inspection: 667099.015 - Allied Genetics, Llc |

Anthony M. "Tony" Gabriele, 49Tony was fatally injured on the job in January 2015 while working for Silvi Concrete (Riverside) in Bristol, Pennsylvania. He was fatally injured when a silo collapse and he was buried under mounds of cement. On January 11th Tony was found after being buried for three days.

Dustin Wayne Galyan, 25Dustin was fatally injured on the job in February 2015 while working for Texoma Power Line Company in Jones, Oklahoma. He was fatally injured while working in an extended bucket attempting to put an armor guard on the line and was electrocuted.
Federal OSHA investigated the incident and proposed a $14,000 penalty with three serious violation. This case is still open. Inspection: 1040868.015 - Texoma Powerline, Inc. My son Dustin Wayne Galyan he was born 02-26-1989 he was 25 yrs old. He lived in Jones, Oklahoma. He died 02-13-2015, he was a lineman and got electrocuted on the job. Dustin leaves behind a 15 month old son, mother, father 2 sisters and 4 brothers, and tons of family and friends... He was a great guy, who loved his family, he was the type of guy that never met a stranger, he would give you the shirt off his back and never ask you for anything in return.. He is loved and missed by so many.... Thank you, for inviting us to the workers Memorial day candlelight vigil, it meant a lot to me. Dustin is also survived by an unborn daughter, who is due in August.

Nicholas Gamalski, 27
Nicholas was fatally injured in November 2017 while working for Irby Construction in Sulphur, Louisiana. Nick was working on power line with his fellow co-worker when their harness line rubbed against a power line, causing it to break. Nick and his co-worker fell approximately 100' from the helicopter fatally injuring both men.
Inspection: 1277788.015 - Irby Construction Company Nick was a very well loved man. A man who would give the shirt off his back. he loved spending time outdoors hunting and fishing with friends, conversations around a fire with a beer or cocktail in his hand. hard worker, ambitious and thrived to be great, which he was... His legacy is lived on through family and friends. |

Whitney Gardner, 30
Whitney was fatally injured in March 2020 while working for LFUCG- Division Of Waste Management through a Temp Agency in Lexington, KY. She was found unresponsive on the ground behind the garbage truck.
KYOSHA investigated the incident and proposed a $7,000 penalty for one other than serious violation. In an informal settlement agreement the citation was reduced to $3,500.00. This case is closed. She was on the job, when a horrible incident occurred that led to her death. March 27, 2020 is a month and date I'll never forget. Cody Gillespie, 23
Cody was fatally injured in October 2019 while working for Waste Pro Of North Carolina, Inc. in Swannanoa, North Carolina. He was fatally injured after being thrown from the garbage truck to which he was riding on as it swerved off the road to avoid a vehicle that had crossed over the center line.
NCOSHA investigated the incident and proposed a $75,000 penalty with one serious violation and one other than serious violation. This case is still open. Inspection: 1442091.015 - 143929 - Waste Pro Of North Carolina, Inc. "He was only 23 . Just starting his life. Survived by his mother, 1 aunt, and 2 older brothers . 1 niece and 2 nephews, and a sister n law. Its EXTREMELY hard every single day without my baby boy. A part of me died that day with him. " |

Eric Gohagan, 44Eric was fatally injured in December 2017 while working for Pipeline Systems, Inc. in Valley Grove , West Virginia. He was fatally injured when an excavator slid on ice and hit him.

Raymundo C. Gonzalez, 54Ray was fatally injured on the job in November 2004 while working for BP in Texas City, Texas. He was fatally injured in a super heated steam explosion. Leonard Maurice Moore was also fatally injured in this incident.
Federal OSHA investigated the incident and proposed a $109,500.00 penalty with seven serious and one willful violation. In an informal settlement the penalties were reduced to $102,500.00 for six serious violations and one willful violation. This case is closed. Inspection: 306482266-Bp Products North America, Inc. Less than 6 months later, another explosion occurred at this same worksite, killing 15 workers.

Matthew Goodner, 30Matthew (from Iowa) was fatally injured on the job in October 2011 while working for American Tower Corp. in Newton, Massachusetts. He was fatally injured when fell 1,000 feet while replacing an antenna pole and safety ladder on the tower.

James Grahame, 52James was fatally injured while working for SBC (now AT&T), CWA Local 9410 in San Francisco, CA. He died when he contracted mesothelioma as a result of exposure to asbestos while performing his telecommunications work.

Landon D. Gran, 18Landon was fatally injured on the job in August 2019 while working on a rural Norseland farm in Norseland, MN. He was fatally injured after making contact with a piece of equipment which was believed to have been an auger while in a grain bin.
This case was not inspected by OSHA as they had no jurisdiction. |

Margaret Gray, 46Margaret was fatally injured on the job in November 2016 while working for Adams Construction near Greeley, NE. On October 3, 2016, Ms. Gray was performing duties as a flagger on the south side of the road construction project. A vehicle driving approximately 60MPH approached the construction zone and struck Margaret who was positioned on the shoulder of the road and out of the normal traffic flow. She was thrown from the point of impact approximately 140 feet and coming to rest approximately 12 feet from the road surface on the shoulder. Ms. Gray succumbed to her injuries on November 15, 2016.

Mark Allen Gray, 27Mark was fatally injured on the job in July 2009 while working for Catenary Coal/Sample Mine (Hawkeye Contracting) Company in Eskdale, West Virginia. He was fatally injured when he was in a truck rollover and drowned in a pond.
MSHA investigated and issued one citation; a civil penalty has not yet been assessed by MSHA. Catenary Coal Co. Tribute by his wife Bevery McCarty Gray: Mark's life was tragically ended working as a surface miner in WV when his articulated truck rolled into a sediment pond, entrapping him and he drowned. Mark was a very compassionate person who loved life and was loved by many. He loved children and for years, along with his wife, taught Sunday School for several years. Mark was an avid fisherman and loved to hunt. He was a beautiful person, inside and out and many lives were changed due to this preventable accident.

This is my son Jason E. Gudat born November 19, 1979 (age 31) has twin sister Jaymie Lynn Gudat Andrews died April 25, 2011 E. Fairfield Coal (Subtropolis) Petersburg, Ohio. I am Jason's mother Karen A. Harris, my husband Arden F. Harris Sr. his father Ronald Gudat grandmother Mildred Loudon he has a twin sister Jaymie Lynn Gudat Andrews a sister Stephanie Ann Gudat Burns step brothers Clyde Harris Arden Harris Jr. Step sisters Elaine Clark Teresa Bostwick Crystal Mounts Joy Shawver many nieces and nephews.

Jenny Gulledge Gabriel, 61Jenny was fatally injured on the job in June 2020 while working at Weiser Security Services in Salisbury, NC. She was fatally injured when a tractor trailer struck her.

She wasn’t a push over at all but if she could help you, she would find a way to. She was the primary care giver for her elderly Mother & a big supporter of the American Red Cross; Momma donated blood routinely. It was just a normal Thursday. She went to work and just never came home. When it first happened, I thought not having the opportunity to say good-bye was going to be the toughest part but I was wrong. It’s been almost a year now; the hardest part is knowing that she had so much more life to live, all the milestones she isn’t here to celebrate. That’s the hardest part.
