Michael Fernandez Haith, 36Michael was fatally injured on the job in September 2019 while working for Precoat Metals, A Division Of Sequa Corporation in Hueytown, Alabama. Mr. Haith was struck by a coil that was being lifted by a crane operator. He was then crushed and fatally injured after being caught between the two coils.
Federal OSHA investigated the incident and proposed a $13,494.00 penalty for one serious violation. In an informal settlement the penalty was reduced to $10,121.00 for one serious violation. The case is closed. Inspection: 1431195.015 - Precoat Metals, A Division Of Sequa Corporation I lost my brother Sept Friday the 13th 2019. Due to job negligence. This SCAR will be with my family and I forever. Going to work shouldn't result in a funeral arrangement. This has forever changed my life my only brother gone, just minutes after talking with our mom.
I hope no one ever have to endure the pain and the illness associated with a loss such as tragic as we are experiencing every day of our lives. |

Bruce Hall, 44Bruce was fatally injured on the job in August 2013 while working for Ricks Auto Body in Missoula, MT. He was fatally injured when he was pouring lacquer from a barrel to a smaller container through a funnel and the fumes exploded.

George "Junior" Hamner, 54George was fatally injured on the job in January 2006 while working for Sago Mine, (ICG) in Sago, West Virginia. He was fatally injured in a mine explosion and suffered carbon monoxide poisoning.
MSHA investigated the disaster and did not identify any contributing violations and assessed no penalties. George "Junior" Hamner, 54 passed away on 1/2/06 at the International Coal Group's Sago Mine in Sago, WV. He and 11 other men died from carbon monoxide poisoning.

Christopher James HansenChristopher was fatally injured in February 2006 while working at Jennie-O-Turkey in Melrose, Minnesota. He was fatally injured when a piece of equipment crushed him.

Monique Harper, 41
Monique was fatally injured on the job in August 2010 while working for Producers Rice Mill in Stuttgart, Arkansas. She was fatally injured when she was engulfed by rice.
Federal OSHA investigated the incident and assessed a $6,300 penalty for one serious violation. The penalty was not reduced. Inspection: 314737073 - Producers Rice Mill, Inc. Monique was found buried in a rice storage facility on Tuesday. The Stuttgart Fire Department received the call about 5:30 a.m. When fire crews arrived on the scene, co-workers were attempting to dig the trapped employee out. Fire crews eventually located Harper. Paramedics tried to revive Harper, but she was pronounced dead at a local medical facility at 6:28 a.m. According to a police report Harper's co-workers became concerned about her whereabouts when they were unable to contact her by radio. That's when employees began searching for Harper. Killed at Producers Rice Mill in Stuttgart, AR.

Jared Lee Helton, 22
Jared was fatally injured on the job in November 2019 while employed with A&A Safety working for Tennessee Department of Transportation in Memphis, TN. He and co-worker were struck by a tractor/trailer truck that was hauling freight, at which time it crashed into their mobile truck and the construction zone fatally injuring Jared and his co-worker.
Inspection: 1443159.015 - A & A Safety, Inc. TNOSHA investigated the incident. No citations were assessed. Jared was a beloved son, a brother to Bradley and CasSandra a Uncle to Aria and Elli. He has so many wonderful friends that truly loved him. Jared’s nickname was "Smiley" because he was always smiling. Jared was always happy and had a wonderful laugh. After meeting Jared you had a special friend for life. We will truly miss him and our hearts are forever broken. Such a short life but a big legacy he has left behind. His family created the Jared Lee Helton Good Character Memorial Scholarship program on his behalf. This way Jared is still helping others pursue their goal as he was working on his. |

Everett James Henry, 25
Everett was fatally injured on the job in September 2022 while working for Father Flanagan's Boys Home in Boys Town , Nebraska. He was fatally injured when he was driving a ride-on mower while mowing grass on a peninsula of a lake. He was near the lake and ended up falling in and getting caught and drowned.
Federal OSHA investigated the incident and assessed a $10,360 penalty for one serious violation. In an informal settlement the penalty was reduced to $9,000 for one serious violation. Inspection: 1621343.015 - Father Flannagan?S Boys? Home Everett was an amazing son! He had two younger brothers and a mom and dad who miss him so much! He had his associate's degree in horticulture and while working full time at Boys Town, was taking online classes to get his Bachelor's in Urban Forestry through Oregon State University. He had achieved his Pesticide Applicator's License and also his Nebraska State Arborist's license and the International Society of Arboriculture certification. He was very proud of those certifications. He also was learning how to make Grandma Henry's pickles using her recipes, loved photography and taking pictures of stars, had just gotten his drone license and loved spending time with his best friend, Jessica. The photo we chose to honor Everett was a selfie taken by him after following clues and climbing up to get the prize in a tree from Treestuff.com. He was so proud that day! We miss him so much! Losing Everett has been the most difficult thing our family has faced, including his dad fighting prostate cancer. This should never have happened - Everett should still be here watching reruns of Psych with us and enjoying driving his 1972 VW Super Beetle and going on more adventures with Jessica. |

Gregory Hock, 65
Gregory Hock, 65 was fatally injured after working over 30 years for Carpenter Technology Corporation in Reading Technology, PA. Mr. Hock was diagnosed with stage 4 metastatic cancer, bladder, liver and bone. Within 2 months Gregory had lost over 100lbs and had to use a walker for assistance. He sadly succumbed from his illness approximately 2 months after being diagnosed.
This is out of OSHA's jurisdiction. However it is believed that Mr. Hock as a Grinder for over 30 years without being provided with personal protective equipment was exposed and in contact with toxic chemicals such as biocide (a chemical directly in his machine), asbestos and hex chromium. I hope that OSHA takes these toxins seriously so no other worker or family suffers. My father was not the first, he was one of several in his department to pass from this. Masks and other safety equipment must be worn at all times. I will forever fight for my father. He sacrificed seeing me grow up to make sure we had by working 7 days for over 30 years. Tommy Joe Holley, Jr. 53Tommy was fatally injured on the job in October, 1996 after working on towers for 35 years. He contracted Acute Radiation Syndrome (ARS) and suffered stochastic effects of radiation.
This is not in OSHA's jurisdiction. Tommy (of Bridge City, TX) passed away from too much radiation exposure working in the tower business. This exposure causes nausea, headaches, and it affects the heart, lungs, and kidneys. He was a “towerdawg” for 35 years. Tommy worked for 11 years on Radio Towers and 24 years on Cell Towers.

Sherman Lynn Holmes, 55Sherman was fatally injured in February 2011 while working at K&K Forest Products in Evart, Michigan. He was fatally injured when struck by a felled tree.

Our dad was a many of few words but when he did speak those who were around him listened because they knew it was important. As his daughters we feel his biggest pride was his grandchildren. He promised them he would teach them to hunt and fish when they were older and sadly he never had the chance. Our dad was with us when and where ever we needed him. He traveled hours for his first grandchild Leslies open-heart surgery and as she passed he was with his family. He made the trip to Ohio when his next two granddaughters Lauryn and Kynndal were born, and to Alexandria Virginia when his first Grandson Jackson was born. He also made the trip when Granddaughter Gabriela was born and was the first one to the hospital when she had her first open-heart surgery. Danielle was still pregnant with the youngest granddaughter Jillian, when dad was killed. ----If asked to describe one of our fathers greatest joys (besides family) we all agree coffee and a doughnut would be a top priority. Our lives will never be the same and we miss him everyday."

Stephen B. Houston II, 27
Stephen was fatally injured on the job in February 2020 while working for Academy School
District #20 in Colorado Springs, Colorado. He was fatally injured while doing snow removal for his employer. This was not in OSHA's jurisdiction. "Stephen did not know his pick-up truck was grossly overloaded with sand and salt, by over 1000lbs. The Salty Dog sander was improperly installed to the frame of his truck and the cotter pins to secure the sander grate to the sander was not used. There were no headache bars to protect Stephen from the sander and grate causing it to come crashing in the back window when he spun on the ice. Stephen was killed immediately. We were informed that the crash was survivable, but what was not survivable was lack of concern for his safety." "Loved family, friends and all things Broncos, Avalanche and Jeeping. Federal OSHA investigated the incident and proposed a $7,000.00 penalty for one serious violations to Barnhart Crane And Rigging, Inc. In an informal settlement the penalty was reduced to $3,500.00 for one serious violations. The fines were paid and the case is closed. Inspection: 314557000 - Mccord Communication Service, Inc Inspection: 314557018 - Barnhart Crane And Rigging, Inc. Jonce Hubble was doing maintenance on a communications tower in Anniston, AL on 7/22/2010. The tower's guy wires were clipped by a 33 foot bucket truck that was moving around the worksite. Jonce passed away around 9.00 pm at UAB in Birmingham after several surgeries and two amputations.

"Butch" Elmer Clayton Hulse, 59Butch was fatally injured on the job in August 2011 while working for Union Pacific Railroad in Saline, Kansas. He was fatally injured when his truck became stuck on the tracks and a car struck him.
No Investigation Information found 9/18/2013. Butch was killed while working at Union pacific Railroad as he was on the rail truck inspecting. He was going through a crossing when a woman hit him knocking his truck off the track, the truck rolled and he was thrown from it.

Ossie L. Hyman, 47Ossie was fatally injured in October 2011 while working for Port Of Miami River Terminal, LLC. in Miami, Florida. He was fatally injured when the platform he was standing on failed, he fell approximately 18 feet and a container crushed him.
Federal OSHA investigated the incident and proposed a $14,700 penalty and three serious violations. In an informal settlement the penalty was reduced to $11,025.00 for one three serious violations. The company paid the fines and the case is closed. Inspection: 315357665 - Port Of Miami River Terminal, Llc. Employed at the Port of Miami for 21 years, and fatally injured when he was unloading a container on a ship called “The Spirit” when the container crushed him.
