Eric Jeppesen, 44
Eric was fatally injured in January 2022 while working at Industrial Cork Company, Inc. in Chicago, Illinois. At 8:35 a.m. he was laying on a furniture dolly and rolled along a suspended ceiling made of insulated metal panels to retrieve a battery. While rolling along the ceiling tiles, the employee fell from an edge of a panel. The employee was transported by ambulance to the hospital to treat unspecified, blunt force injuries, but later passed.
Federal OSHA investigated and proposed a $21,132 penalty for one serious violation and one repeat violation. In an informal settlement the penalty was reduced to $17,800 for one serious and one repeat violation. This case is still open. Inspection: 1571577.015 - Industrial Cork Company, Inc. Eric was an amazing dad to his 2 sons he left behind. At the time of his death Eric Jr. had just turned 12 and Roger was 8. He was their best friend and confidant. They loved celebrating Halloween and the 4th of July together. Eric was also my high school sweetheart we started dating when I was 15 and he 18. We made it 26 years together, 15 of those happily married, he was also my best friend. He lived for Pink Floyd, rock concerts, and his 1957 Chevy Bel Air. We will never be the same without him and miss him dearly every single minute!! |

Keith Douglas Jewell, 44Keith was fatally injured in August 2019 while working at Kentucky Department of Transportation in Campbellsville, Kentucky. He was struck by a vehicle in construction zone.
He left behind 2 children Zachary and Carrington. |

Brian Johnson, 43
Brian was fatally injured on the job in September, 2017 while working for General Dynamics in Abingdon, Virginia after suffering a cardiac arrest due to overexposure to EMF Activity.
Inspection: 1261022.015 - General Dynamics "My husband went into work that day for a 12 hour shift working in radome testing in an anechoic chamber in an electromagnetic field. Near the end of his shift, he collapsed convulsing and sweating due to cardiac arrest. The electric part of his heart that controlled the beating was effected and he went into arrest not due to a heart attack. His employer did not call 911 immediately and there was a delay in getting an AED to him that was on the premises. When paramedics finally arrived they were able to get a pulse, but my husband died five days later from no brain activity caused from him being without oxygen due to his negligent care and overexposure to EMF activity. Nicholas “Nick" Zachary Johnson, 21Nick was fatally injured on the job in December, 2012 (the same day as his birthday) while working for South Carolina Department of Transportation (SCDOT) in Lexington County, South Carolina. He was fatally injured when he was struck by a hit and run driver.
SCOSH investigated and proposed $0 penalties and no violations. The case is closed. Inspection: 316621499 - SCDOT Nicholas Z Johnson was my son. We was born Dec 3 1990 in Chicago IL. He attended Jacobs high school in Algonquin IL where he loved sports . He played football, wrestling, baseball and swimming. He worked for SC Department of Transportation. He lived his job and the men and woman he worked with. He also loved his family and spent a lot of time and memories with us. He was killed on Dec 3 2012 by a DUI motorist. Dale Jones, 44Dale was fatally injured in December 2009 while working for Anderson Indiana Street Department in Anderson, Indiana. He was fatally injured when he was struck by a car picking up debris.
IOSHA investigated and issued proposed no penalties or citations. Inspection: 313197659 - City Of Anderson Dale was picking up debris from a road after dark and was struck by a car traveling at 50 mph while he was working for the Anderson Indiana Street Department. While working at Anderson Indiana Public Works (Street Department) in Anderson, Indiana. He died after being struck by a car.

Steven Kyle Jones, 42Steven was fatally injured while on the job in January 2012 while working for Energy Drilling Company in Longview, TX. Steven was fatally injured when he fell 50 feet from a derrick rig.
Federal OSHA investigated and proposed a $6,300.00 penalty for one serious violation. These were not reduced. Inspection: 316093632 - Energy Drilling Company. Stevie was born March 20,1970. He died on Friday January 20, 2012. He was working for Energy Drilling Company out of Natchez, MS. According to the only report we have seen (why by the way was in the Longview, TX newspaper) Stevie fell 50 feet from the derrick to the floor of the rig. He was working on Rig number 1, for Energy Drilling Company, Natches, MS on Rig #1 located near Atlanta, TX. My husband and I are working on this together, and we were his brother and sister in law. Undre Deon Jones, 42Undre was fatally injured on the job in September 2012 while working for FTS International formerly Frec-Tech Services in Cotulla, Texas. He was fatally injured when he was struck by a valve from fracking manifold system that he was working on.
Federal OSHA investigated the incident and proposed a $14,000.00 penalties for two serious violations. The case is still open. Inspection: 649898.015 - Fts International, Inc. My husband Undre Jones was an employee of Fts International formerly known as Frac-Tech. Undre is from Shreveport,Louisiana. My husband was killed September 18,2012 while working. He leaves behind a loving wife Cordonna, two beautiful daughters Di'ana Jones(3 yrs.), Dre'ci Jones(1yr.), step daughter Coreshia Byrd. We all miss him more and more each day, wishing and imagining his presence. Throughout the healing process I've learned to "Live One Day At A Time". No one will ever replace our loving father, husband, friend.

Leilani Margurite Jordan "Butterfly", 27Leilani Jordan became ill on the job in March 2020 while working for Giant Grocery Store in Largo, Maryland. She is believed to have contracted the COVID-19 virus while at work.

Nicole Chartre' Joslyn, 24Nicole was fatally injured on the job in October 2004 while working for Arby's in West Milwaukee, Wisconsin. She was shot by a disgruntled employee.
This is not in OSHA's jurisdiction. Nicole Chartre' Joslyn, 24 was killed on the job on 10/24/04 while working at Arby's in West Milwaukee, WI. She was murdered by a disgruntled employee. Murdered by a coworker and a disgruntled former employee in a planned robbery and intentional double homicide. She was found in the Arby's cooler/freezer an hour or so after closing time; shot multiple times. |
