Kevin Keane, 28Kevin Keane was fatally injured in July 2008 after working for Single Ply Systems in Saint Paul, Minnesota. He was fatally injured after plunging threw a skylight and trying to recover from his fall.

Nolan Keane, 18Nolan was fatally injured in October 1999 while working for Single Ply Systems in Saint Paul, Minnesota. He was fatally injured when the wind blew catching the insulation, sweeping him across the roof, plunging him 45 feet through a Plexiglass skylight.

Corey A Keller, Sr., 41Corey was fatally injured on the job in July 2010 while working at Horsehead Corporation in Monaca,Pennsylvania. He was fatally injured when oxygen from the outside air caused the vaporized zinc to combust resulting in a fire ball.

Paul Robert King, 50Paul was fatally injured on the job in July 2005 at Boston Logan International Airport while working at MainTech, LLC. in Boston, Massachusetts. He was electrocuted while working on a piece of equipment.
Federal OSHA investigated and proposed a $55,000.00 penalty for eight willful violations. The case is closed. Inspection: 308108885 - Main Tech. He was assigned the job of a licensed electrician even though he held no licenses. His Company was held liable for the full amount because they filed an appeal too late. William E. (Willy) Kingrey, 44William was fatally injured on the job in December 2010 while working at Asplundh Tree Expert in Trenton, Ohio. He was fatally injured when he was struck by a CSX train.

Michael R. Kittle, 29Michael was fatally injured on the job in February 2014 while working at Wieser Concrete in Portage, Wisconsin. He was fatally injured when chains holding a cement block gave way and crushed him.

Larry Kizer, 62Larry was fatally injured on the job in December 2019 while working at Louisville Jefferson County Metro Government in Louisville, Kentucky. Mr. Kizer was found unresponsive pinned underneath his tow truck.
Kentucky OSHA investigated this incident. This case is currently open. Inspection: 1453722.015 - 61353 - Louisville Jefferson County Metro Government My father truly was my hero. He had a strong spirit and tried to give me my sister and his family the best world possible. He was a great grandfather who spent many weekends with his oldest grandson at Holiday world. He never skipped a beat with his children, family or grandchildren. He loved animals and would feed local stray cats around the tow lot and on the street. He truly is a one of a kind. Gone so soon with so many unanswered questions. A Tragic death it’s never easy. |

William Knorr, 32William was fatally injured in June 2004 while working at Grand Tower in St. Ignace Township, Michigan. He was fatally injured when he fell from a cell tower.
OSHA investigated the incident and proposed a $98,000.00 penalty for four serious violations and one willful violation. In an informal settlement they were reduced to $20,500 for two serious violations and three other violations. Inspection: 307697490 - Grant Tower Inc. Michigan. |

Travis Wayne Koehler, 26
Travis was fatally injured in February 2007 while working for Orleans Hotel Casino/Boyd Gaming in Las Vegas, Nevada. He was fatally injured after attempting to rescue his co-worker in a confined space.
NV OSHA investigated the incident and proposed a $185,000.00 penalty for eight serious violations and one other violation. This penalty was not reduced. Stephen D. Kuckta, 65
Stephen was fatally injured on March 2020 while working for UPS in Omaha, Nebraska. He was fatally injured after sustaining injuries when he fell off of a dock. The injuries are believed to have caused him to go into cardiac arrest a few days later at his home.
The incident was not inspected by OSHA. |
