Kenneth John Maggert, 58
Kenneth was fatally injured on October 2020 while working for Quality Springs Togo, Inc. in Coldwater, Michigan. He was fatally injured when he was crushed in a shot blast machine.
MIOSHA investigated the incident and proposed a $21,000 penalty for three serious violations. This case is closed. Inspection: 1499406.015 - Quality Spring Togo Inc Ken was a devoted and loving husband, father, Papa, son, brother and friend to many. He had a kind heart and would always help you out if needed. He enjoyed his family, (especially his grandchildren), fishing, going to old car shows and listening to music. I miss my husband. Russell Mann, 45Russell was fatally injured in September 2012 while working for Wynnewood Refining Company (CVR Energy Inc.-owned plant) in Wynnewood, Oklahoma. He was fatally injured when a boiler exploded. Co-worker, Billy Guy Smith, 34 was also fatally injured in the same incident.
Federal OSHA investigated the incident and proposed a $234,500.00 penalty for eleven serious violations, five repeat violations and one other than serious violation. The case is still open. Greg Martin, 44Greg was fatally injured in September 2008 while working for B.L. Myers Bros. in Glenmoore, Pennsylvania. He was fatally injured when a 34-ton vehicle tipped onto its right side and he was hit by the 25-foot mast.

Orestes Martinez, 29Orestes was fatally injured on the job in April 2009 while working for J.T. Vaughn Enterprises in Houston, Texas. He was crushed when a two-ton door fell on him while installing it at M.D. Anderson Cancer Center.

Benjamin "Joby" Mast, 30Benjamin was fatally injured on the job in May 2014 while working at Reading Alloys for Z Weldco in Oley, Pennsylvania. He was welding an industrial hopper filled with lime that became unstable and fell on him.

Mark Mattern, 59
Mark was fatally injured in September 2022 while working for Century Casting Corporation in Swansea, Illinois. He was fatally injured when he was assisting a coworker to troubleshoot an issue with a Roberts Sinto Corporation FBO Series Molding Machine, serial number FMM-0106-00. The machine was incorrectly forming molds due to excessive sand and debris underneath the hydraulic mold lift section. Without shutting the equipment down, de-energizing, or locking the equipment out, the Mark and the coworker used shovels and brooms to clear the sand. The hydraulic mold lift was still not moving into the down position, so he crawled underneath to clear additional sand by hand. While still under the lift section, the lift came down fatally injuring him.
Federal OSHA investigated this incident and proposed a $32,590 fine for four serious and one other than serious violation. In an informal settlement the penalty was reduced to $21,564 for four serious and one other than serious violation. This case is currently opened. Andrew Joshua Dean May, 35Andrew was fatally injured in June 2019 while working for Lone Star Truck And Tire Inc. in Brooklyn, Iowa. He was fatally injured after patching a steel radial tubeless semi-trailer tire and mounting it on a steel wheel. At which time a bead failure on the inside dual caused a release of air that forced the outside wheel to strike him in the head and chest. He was then knocked backwards approximately 13 feet, striking the rear area of another semi-trailer.

Kyle McCullough, 27
Kyle was fatally injured in March 2023 while working for Sooner Towing and Recover in Shawnee, Oklahoma. Kyle was fatally injured when he had just finished strapping the vehicle down that he was towing. While he was walking to the controls to finish up he was struck by a semi. The semi driver did not stop until they were stopped further down the road. Kyle did have all of his gear out and his lights on at the time.

Mitchell Eugene McDaniel, 19
Mitchell was fatally injured in November 2019 while working for Vandoske Dairy Farms LLC in Cleveland, Wisconsin. Mitchell was fatally injured while working all by himself inside an old concrete silo, while an exposed and completely unprotected & unguarded auger was running the entire time he was inside the silo.
Mitchell's incident was not inspected by OSHA as it was out of their jurisdiction. A life lost is still always a precious life lost and every single incident and death in the workplace deserves a thorough and proper investigation by the properly trained authorities who specialize in this area. Mitchell had a personality that was larger than life. You could be having the worst or cruddiest of days and just his goofy antics, regular dance moves & jiving around the house or him displaying that mischievous & half crooked little smile he always carried around.....he would brighten up your day. He was one of those young men who always stood up for what he believed in, always stood up for the underdog, and he was most definitely always there for his buddies. He is loved beyond measure & greatly missed every single day by his parents, younger brother, older sister, entire family & everyone who knew him. Michael McKee, 27Michael was fatally injured in December 2016 while working for a Temp Agency at Texamerican Food Blending, Inc. in Hot Springs, Arkansas. He was fatally injured while inside of a mixer when the emergency stop button was energized trapping him inside the mixer causing trauma to his chest.

Stacy Lynn (Walls) McKinney, 27Stacy was fatally injured July 2006 in Tulare, NY when assaulted by her employer.
This is not in OSHA's jurisdiction. FAUSTON BASSO SENTENCED TO 27 YEARS AND 8 MONTHS IN STATE PRISON -On July 20 2009, Tulare County Superior Court Judge Darryl Ferguson imposed a prison sentence of 27 years and 8 months in state prison on 39-year-old Fausto (Mike) Basso of Garden City, New York. On June 4, 2009 the defendant plead no contest to voluntary manslaughter, assault with a deadly weapon, and residential burglary. He admitted that he personally used a firearm during the crimes and that he inflicted great bodily injury. The defendant killed victim Stacey McKinney age 28 on July 13, 2006 in her home in Tulare. The defendant also inflicted serious injury on victim Andrew McKinney age 29, during the attack. All three crimes are violent felonies or strike offenses under the 'Three Striker' law. Furthermore, his credits are limited to 15% on the entire case. He was ordered to pay over $11,000 in restitution and over $1000 in fines. This case was prosecuted by Deputy District Attorney Afreen Kaelble, and investigated by Detective Todd Davis of the Tulare Police Department and Investigator Brain of the Tulare County District Attorney's Office Bureau of Investigations. |

Cody Earl McMullin, 32Cody was fatally injured in January 2011 while working for Nine Mile Canyon in Roosevelt, Utah. He was fatally injured when he was crushed by machinery.
January 30, Nine Mile Canyon UT - 32 year old Cody Earl McMillan of Roosevelt, a water truck driver, had been removing water from a pit at a location in the Wells Draw area of Nine Mile Canyon when he climbed onto the pump jack at the well site for some reason, said Duchense County Sheriff's Sgt. John Crowley. McMillan fell into the pump jack and suffered fatal crush injuries, Crowley said. The man's body was discovered by other oil-field workers when they arrived at the site Sunday morning.
