Glen Steven Rackley, 51Glenn was fatally injured in November 2011 while working for Danco Construction in Evansville, Indiana after falling approximately six feet from a scissor lift.
IOSHA investigated the incident and proposed a $14,700.00 penalty for six serious violations. In an informal settlement the penalty was reduced to $6,000.00 penalty for four serious violations. This case is closed. Inspection: 315677120 - Danco Construction Inc. |

Burton Issac Reimer, 17Burton was fatally injured while fishing on Lake Winnipeg (Canada).
Issac Reimer died while commercial fishing on Lake Winnipeg October 23, 1999. He was 17 years old. I am his mother and a board member with Association for Workplace Tragedy Family Support in Canada. Better known as www.threadsoflife.ca Each worker and family member who has been affected by a workplace tragedy is a thread in the quilt of life. Each thread, by itself, cannot stand alone, but when woven together provides strength. Although we are individuals, we are also connected in the fabric of life. Dirk Remington, 46 Dirk was fatally injured in December 2009 while working for Demand Communication, Inc. in Owego, New York when the tower he was working on collapsed.
OSHA investigated the incident and proposed a $2,400.00 penalty for one serious violation and one other than serious violation. The penalties and violations were not lowered, This case is closed. Inspection: 312372824 - Demand Communications, Inc. one serious one other than serious. They were in the process of replacing guy wires in a maintenance job. The tower collapsed at approximately 2:30 in the afternoon and the outcome of the OSHA investigation has not been completed. Also Sent by Daughter Denny Remington, who states he is my father and best friend. |

Christopher Ricci, 27Christopher was fatally injured in December 2014 while working for D.L.S. Prestressed Inc. in Sunrise, Florida. He was fatally injured when he was crushed by a crane traveling over uneven terrain and lost its balance and toppled over.

Oscar Rivera, 53Oscar was fatally injured in August 2013 while working as a subcontractor for Hillpointe Construction Incin in Goleta, California. He was fatally injured when he fell through an unprotected skylight while painting outside of MOOG INC.
CalOSHA investigated the incident and proposed a $15,600.00 penalty for two serious and four other than serious violations. IN LOVING MEMORY OF Oscar Rivera* OSCAR RIVERA -July 17, 1960 - August 30, 2013 ~53 years old~ Oscar Rivera was killed on the job on August 30, 2013. He was painting the outside of MOOG INC. in Goleta, ca. He was on the roof when he fell through a skylight. He fell 25 feet down to his death. The contractor that he was working for did not provide Appropriate Guarding and Fall Protection, when working around skylight. He was a Son, a Brother, a Uncle, a Cousin, and a Father. He left to do a job and he said he would be back in 2 weeks. He never return home. I find my self looking for him, then I realize he is no longer here. His death could of been prevented. His death has change me. my brother was not the first or the last. I will be my brother's voice and for other's that was killed on the job. I remember when we were little and played in the yard and went inside to sit and play cards. You'd look at me and smile, your smile stretch a mile. I look at the door and wait for you to appear, then i realize you are no longer here. I tell myself that i should of told you not to go. Kyle Rhodes, 28Kyle was fatally injured in May 2011 while working for Fairfield Manufacturing in Lafayette, Indiana after being pinned between a forklift and a semi-truck.
IOSHA investigated the incident and proposed a $25,000.00 penalty for 9 serious violations. In an informal settlement the penalty was reduced to $14,500.00 for three serious violations. Zachary “Zach” Aaron Roberts, 25Zach was fatally injured on the job in July 2013 while working for Monarch Towers, Inc in Bismarck, North Dakota. He was fatally injured when he fell approximately 250 feet from a cell tower. Co-worker Namon Smith, 42 fell onto Zach and both perished in the fall.
Monarch Towers in Bismarck, North Dakota was subcontracting for Basin Electric Power Cooperative and the tower is owned by Mountrail Williams Electric Cooperative. Federal OSHA investigated the incident and proposed a $14,000.00 penalty for 2 serious violations. In an informal settlement the penalty was reduced to $5,600.00 for 1 serious violation. This case is closed. Inspection: 919441.015 - Monarch Tower, Inc. Zach was the baby of 4 kids. He was a father to 3 girls and a son. He was also a husband. Zach has always been a comedian and had a beautiful smile. That smile always was present when working or talking about towers. He has worked in the tower industry for 7 years, a good bit with my husband Matthew, whom Zach considered his father. |

Joseph “Joe” Robertson, 22
Joseph was fatally injured in June 2022 while working for A-C Communications Corporation in Montipelier, Kentucky. He was fatally injured when him and a coworker were unloading 30-foot utility poles from a utility trailer. One of the poles fell and struck the him in the chest and head pinning him to the ground. He had passed at the scene from blunt force trauma and skull fractures.
OSHA investigated the incident and proposed a $22,050 penalty for 3 serious violations and 1 other than serious penalties. In an informal settlement the penalty was not reduced. Inspection: 1603489.015 - 65084 - A-C Communications Corporation Joe had many friends and is missed daily by everyone who knew him. I miss him more every day. |

Eric A. Rodman, 30
Eric was fatally injured in January 2014 while working for Andersons White Pigeon Grain in Elkhart, Indiana. He was fatally injured after he fell into grain-car being loaded with grain.
MIOSHA is investigating and proposed a $7,000 penalty for one serious violations. In an informal settlement the penalty was reduced to $3,500. This case is considered closed. Inspection: 315409490 - Andersons White Pigeon Grain Eric was killed January 24th when he fell into a rail car while grain was being loaded. Eric was a devoted husband and the father of a 9 year old little girl and 18 month old little boy. Eric served in the United States Army and completed two deployments and was forever a Momma's boy. |

Vicente Enrique Rodriguez, 20
Vicente was fatally injured in 2009 while working for Hollywood Theatre MGM (Rhino Las Vegas, Llc) in Las Vegas, Nevada. He was fatally injured after falling approximately 36 feet from a makeshift catwalk.
Nevada-OSHA investigated the incident and proposed a $25,000.00 penalty for seven serious and one other than serious violation. In an informal settlement the penalty was reduced to $4,00.00 for one serious violation and one other than serious violation. This case is closed. Inspection: 313436990 - Rhino Las Vegas, Llc Vicente was killed in in 36 ft. 8in fall, employer failed to provide the required fall protection, and instead provided unsafe oak planking rather than proper catwalks while working at Hollywood Theatre MGM, NV. |

Jose Andonis Ramos Romero, 39
Jose was fatally injured in September 2019 while working for Roof It Right Llc in Louisville, Kentucky. Mr. Ramos was fatally injured after coming into contact with one of the powerlines, shocking him and causing him to fall striking his head on a pile of concrete on the ground.
Kentucky OSH investigated the incident. This case is closed. Inspection: 1431759.015 - 61136 - Roof It Right Llc I am one of his sons I'm the oldest out of five, theirs four boys and one baby girl of two years old. My father was a generous man, would love to help anyone who would ask him for help and those in need. His passion was soccer, he loved being in the fields every time he would play he would totally forget about all the bad of the world. We was with my mother for 20 years they got engaged last year when he tragically passed away my mother name is Elsa Nuñez. My father was the love of her life. Ever since he passed away our life hasn't been the same. |

Michael (Mikey) Rongey, 32Michael was fatally injured on the job in October 2012 while working for Crown Castle tower in Mount Vernon, Washington. He was fatally injured after falling approximately 80ft. from a tower.
DOSH investigated the incident and proposed a $450.00 penalty for two serious violations. In an informal settlement the penalty was reduced to $450.00 for one serious violations. This case is closed. Inspection: 316579259 - Ws Consulting & Construction ---Michael Rongey (Washington State) was killed on the job on January 4th, 2013. He was performing a routine maintenance radio swap for Clearwire on a Crown Castle tower in Mount Vernon, Washington. Mike fell approximately 80 feet while trying to position himself in a location which allowed him to perform his duties in a safe manner. When making his transition around the monopole, the mount he was tied off to had failed and caused him to take a fall. Although his life was cut short he will definitely survive in memory by all of his family and friends. |

Lee Randall Ross, 24Lee was fatally injured in May 2012 while working for Long Brothers Oil Company in El Dorado, Arkansas. He passed away after fumes ignited causing an explosion while he was disassembling an old oil tank. Co-workers Mercil "Shane" Chelette, 38 and Coley Gene Lee, 24 were also fatally injured in the explosion.
