UTAH'S FALLEN WORKERS
These are a few of the men and women that went to work in Utah and were fatally injured never going home to their families at the end of their shift. We remember, honor and recognize these men and women for their dedication and hardwork. We would like to sincerely thank the families that shared their loved one's picture and their tragic loss with USMWF.

Cody Earl McMullin, 32
Cody was killed in January 2011 while working for Nine Mile Canyon in Roosevelt, Utah. He died when he was crushed by machinery.
January 30, Nine Mile Canyon UT - 32 year old Cody Earl McMillan of Roosevelt, a water truck driver, had been removing water from a pit at a location in the Wells Draw area of Nine Mile Canyon when he climbed onto the pump jack at the well site for some reason, said Duchense County Sheriff's Sgt. John Crowley. McMillan fell into the pump jack and suffered fatal crush injuries, Crowley said. The man's body was discovered by other oil-field workers when they arrived at the site Sunday morning. |

Dallas Prokopis, 31Dallas was killed in October 2011 while working for Greene's Concrete Cutting in Utah. He died after being struck by a dump truck.
UOSH investigated the incident and proposed a $3,000.00 penalty for one serious violation. They were not reduced. Inspection: 315873463 - Greenes Inc Dallas was moving traffic barrels and due to miscommunication between workers the road closer was opened for traffic. A dump truck came through and hit my husband. He was taken to the hospital and after five days we had to take him off life support, he sustained severe head trauma and a lot of broken bones. I was 8 months pregnant with our third son when he died. Every day is a struggle for me I miss him so much! I love you Dallas Always and Forever |
